Sunscreen Smarts: Choosing the Safest Protection for Your Child

Summer is a wonderful time for family adventures—days at the pool, hikes in the mountains, backyard barbecues, and long afternoons at the park. While sunshine helps our bodies produce vitamin D and encourages outdoor play, too much ultraviolet (UV) exposure during childhood can increase the risk of skin damage and skin cancer later in life. In fact, just a few blistering sunburns during childhood significantly increase the lifetime risk of melanoma.

The good news is that protecting children from the sun doesn't have to be complicated. With so many sunscreen options on store shelves, however, it can be difficult to know which products are truly the best for both your child and the environment.

Fortunately, organizations such as the American Academy of Pediatrics (AAP), the American Academy of Dermatology (AAD), and the Environmental Working Group (EWG) largely agree on the most important features to look for.

Mineral Sunscreens Are My First Choice

Sunscreens fall into two broad categories: mineral (also called physical) sunscreens and chemical sunscreens.

Mineral sunscreens use zinc oxide, titanium dioxide, or both as their active ingredients. These minerals provide broad-spectrum protection against both UVA rays (which contribute to premature skin aging and melanoma) and UVB rays (which cause sunburn). Modern research has shown that mineral sunscreens protect the skin through a combination of reflecting, scattering, and absorbing ultraviolet radiation at the skin's surface.

For most children, I recommend choosing a mineral sunscreen that contains zinc oxide, either alone or combined with titanium dioxide. Zinc oxide offers the broadest protection across the UV spectrum and is well tolerated even by children with sensitive skin or eczema. The AAP and AAD both recognize zinc oxide and titanium dioxide as excellent choices for pediatric sunscreen.

One downside of mineral sunscreens has traditionally been the white residue they leave on the skin. Fortunately, newer formulations have improved considerably and are much easier to rub in than products available a decade ago.

Why Not Chemical Sunscreens?

Chemical sunscreens contain ingredients such as oxybenzone, octinoxate, octocrylene, avobenzone, homosalate, octisalate, and others that absorb UV energy before it can damage the skin.

These products are effective at preventing sunburn, and it's important to remember that using any sunscreen is far better than using none at all. If a chemical sunscreen is the only sunscreen available, or the only one your child will tolerate, it is still an excellent choice compared with going without protection.

However, several chemical sunscreen ingredients have raised ongoing questions regarding their absorption into the bloodstream, their potential endocrine (hormone) effects, allergic skin reactions, and their environmental impact. Studies performed by the U.S. Food and Drug Administration have demonstrated that several commonly used chemical sunscreen ingredients are absorbed into the bloodstream after normal use. This does not necessarily mean they are harmful, but it has prompted the FDA to request additional safety studies before these ingredients can be considered "generally recognized as safe and effective."

The Environmental Working Group recommends avoiding ingredients such as oxybenzone whenever possible, particularly for young children, because of concerns regarding hormone disruption and allergic reactions. While scientists continue to study these questions, many pediatricians—including myself—feel comfortable recommending mineral sunscreens first, especially when effective alternatives are readily available.

What to Look for on the Label

Choosing a sunscreen becomes much easier when you know what to look for. I encourage parents to keep it simple by selecting products with the following features:

  • Active ingredients: Zinc oxide or zinc oxide combined with titanium dioxide

  • Broad-spectrum protection against both UVA and UVB rays

  • SPF 30 to 50. SPF values above 50 offer only modest additional protection while sometimes giving a false sense of security that leads people to stay in the sun longer.

  • Water-resistant if your child will be swimming or sweating

  • Fragrance-free if your child has eczema or sensitive skin

The Environmental Working Group also recommends avoiding aerosol spray sunscreens whenever possible. Sprays can be difficult to apply evenly, and some of the product may be inhaled, particularly by young children. Creams, lotions, and sticks generally provide more reliable coverage. Sunscreen sticks can be especially convenient for applying to the face, ears, and nose.

Don't Forget the Rest of the Sun Protection Plan

Even the best sunscreen is only one piece of a comprehensive sun protection strategy.

Whenever possible, combine sunscreen with other protective measures, including:

  • Wide-brimmed hats

  • UV-protective sunglasses

  • Lightweight long-sleeved swim shirts (rash guards)

  • Seeking shade during the strongest sunlight hours, generally between 10:00 a.m. and 4:00 p.m.

  • Taking regular water breaks to prevent overheating

These simple measures often reduce UV exposure more effectively than sunscreen alone.

What About Babies?

For infants younger than 6 months, sunscreen should not be the primary method of sun protection. Instead, babies should be kept in the shade whenever possible and dressed in lightweight protective clothing and wide-brimmed hats.

If adequate shade or clothing is unavailable, the American Academy of Pediatrics notes that a small amount of mineral sunscreen may be applied to limited exposed areas, such as the face or backs of the hands. Parents should still avoid prolonged direct sun exposure in young infants whenever possible.

A Word About the Environment

Many parents also wonder whether sunscreen ingredients affect the environment.

Research suggests that some chemical sunscreen ingredients—particularly oxybenzone and octinoxate—may contribute to coral reef damage and can be harmful to certain marine organisms under laboratory conditions. Although scientists continue to study the extent of these effects in natural environments, several coastal regions have already restricted the sale of sunscreens containing these ingredients.

Mineral sunscreens containing zinc oxide or titanium dioxide are generally considered a more environmentally friendly option, especially when formulated without unnecessary additives. While labels such as "reef-safe" or "reef-friendly" sound reassuring, these terms are not currently regulated. Reading the active ingredient list remains the best way to know what you're purchasing.

Application Matters More Than SPF

One of the most common mistakes families make is simply not applying enough sunscreen.

Most children need much more sunscreen than parents realize, and sunscreen should be applied 15 to 30 minutes before going outdoors. It should then be reapplied every two hours, or sooner after swimming, heavy sweating, or towel drying.

Remember that no sunscreen is waterproof. Even water-resistant products eventually wash off and require reapplication.

It's also worth remembering that sunscreen is only intended for exposed skin. Protective clothing often provides even better and more reliable UV protection than sunscreen alone.

The Bottom Line

The perfect sunscreen is the one your family will actually use consistently. Fortunately, today's mineral sunscreens have improved tremendously—they spread more easily, leave less of a white residue than older formulations, and provide excellent protection for children's sensitive skin.

When choosing a sunscreen for your child, I generally recommend a broad-spectrum SPF 30 to 50 mineral sunscreen with zinc oxide as the primary active ingredient. Combined with hats, UV-protective clothing, sunglasses, plenty of shade, and regular reapplication, sunscreen allows children to safely enjoy all the benefits of outdoor play while helping protect their skin for decades to come.

Sun protection isn't about keeping children indoors—it's about giving them the freedom to explore, play, and make lifelong summer memories while taking simple steps to keep their skin healthy for years to come.
