Essentials Membership
Age 0-11 mo
$200/month
Age 1-2 yrs

This plan is designed for families seeking an integrative pediatrician as their primary care doctor for their children. It covers all routine physicals (check-ups) plus up to 6 sick visits per year as needed and text/phone access to Dr. Capella for urgent issues.
There is a one-time enrollment fee of $100 per family which helps cover costs associated with onboarding and reviewing prior medical records. There is also a 6-month minimum requested (which may be waived under extenuating circumstances such as an unexpected move or job loss).
There is a family maximum charge of $400/month for the basic membership plan.
Home visits are available for newborns and children with special needs for an extra $100 travel fee (within 8 miles of the clinic).
Premium Membership
Any Age - $200-$500/month depending on needs
This plan is designed for children who have chronic medical or behavioral health conditions who may need more frequent visits. At the first visit Dr. Capella will help assess your child’s needs and create a custom plan.
There is a one-time enrollment fee of $100 per family which helps cover costs associated with onboarding and getting to know your child’s medical history.
A home visit plan may also be considered depending on needs.
Integrative Consultation Package
$1200 for 3 months of care
For families who want to stay with their primary care physician but also want some extra guidance on integrative treatment options for any condition (including chronic pain, POTS/dysautonomia, PANS/PANDAS) this package includes:
A 90-minute initial consultation
Two 60-minute follow-up visits
Texts and emails during the 3-month period
All visits must be used within the 3-month period. Virtual visits may be substituted for the in-person visits where appropriate.
Patients must have at least part-time residence in Utah or California.
Note: For medico-legal reasons, I cannot provide vaccine waivers under this package.
All Memberships Include
Routine check-ups and sports physicals.
Necessary visits for colds, flus, sore throats, ear infections, skin rashes, or simple injuries
Management of chronic conditions such as eczema, asthma, migraines, acne, and more.
Basic diagnosis and treatment (not including intensive therapy) of behavioral health conditions like depression, anxiety, and ADHD.
Medically-necessary procedures, including stitches, skin glue, and simple abscess drains.
Routine vaccines (billed through your insurance or free for uninsured/Medicaid)
Access to your doctor through text and phone 7 days a week during daytime hours.
Telemedicine visits when appropriate.
In-house testing (including rapid tests for Covid-19, flu, RSV, strep, urine infections, hemoglobin, lead).
Blood tests and imaging (X-rays) are not included, but are available at affordable rates or can be billed through insurance.

Frequently Asked Questions
Why don’t you accept insurance?
Under the direct primary care (DPC) model of healthcare, families pay a monthly membership fee and, in exchange, I serve as the primary care doctor for your children. This takes insurance and its many extra costs and restrictions out of the relationship — which means that YOU get more of my time and attention, whether in the clinic, through a virtual visit, an e-mail, text messages, or on the phone.
Does a membership mean I don’t need insurance?
We still encourage everyone to have health insurance to help cover the high costs of unexpected things — like surgeries or hospitalization. Also, you may occasionally need to consult a specialist (such as a neurologist or ear, nose, & throat doctor) for complex issues, and insurance is usually helpful in those cases. Also, it is helpful to have insurance to help cover the costs of labs, imaging tests, and vaccinations.

What is your clinic schedule?
The clinic is open for routine and urgent appointments on Mondays, Wednesdays, Thursdays from 9:30am - 6pm, and alternating Saturdays from 12pm - 6pm. If your child has an urgent issue, Dr. Capella can usually make special accommodations to see them the same or next day.
Dr. Capella occasionally attends conferences or travel. In these cases, she is usually still available by text and there is another pediatrician (Dr. Diaz-Ochu) that can see your child for urgent illnesses. Members will be notified in advance of prolonged absences. There may be rare occasions when there is no back-up coverage and an urgent care or ER will be recommended during those times.
Where is your office located?
The clinic is located at 3538 S Highland Drive in Millcreek, Utah.
What is your stance on vaccines?
Dr. Capella believes that the risks of vaccines are generally minimal and that following the recommended vaccine schedule benefits children and the population as a whole. However, she accepts families anywhere on the spectrum from non-vaccinating to fully-vaccinating. For medico-legal reasons, families refusing all vaccinations may be asked to sign a vaccine refusal form acknowledging they understand and accept all risks of non-vaccination.
What if my child needs blood tests or imaging tests?
I have contracts with local laboratories (LabCorp) and imaging centers (Mountain Medical) to get affordable rates on blood tests and imaging (such as x-rays, CT scans, or MRIs). A panel of common blood tests might cost about $25-50, and a CT scan may cost about $300. I have a medical assistant who can draw blood in the office, or I can send an order for bloodwork to a lab at Intermountain or Primary Children’s Hospital.
